Athletic Physicals

All students who wish to participate in interscholastic sports MUST have a physical exam. Babylon offers physical exams prior to sports seasons but exams can be performed by a private physician as well. If you do use a private physician you must have them complete the Babylon Physical Exam Form.
 

All Physical Exam Forms are returned to the HS Health Office.
